• Submission fee only — $5.00: Receive the official submission form to complete and return with the participant’s information, biography, photo, consent, and signatures.
• Online book access — $10.00: Receive digital access to the finished 2027 book when it is released.
• Submission + book access — $12.50: Receive the official submission form now plus access to the finished digital book when released.

How submissions work

After purchase, download and complete the official submission form. Return the completed form and photo to suzie@justhalos.com. A parent or guardian must authorize submissions for participants under age 18.
